Now Hiring: Licensed Social Worker | Austin, TX | Skilled Nursing Facility

We're seeking a compassionate and motivated Licensed Social Worker to join a skilled nursing team in Austin, TX . This is an opportunity for a Licensed Social Worker who is passionate about supporting residents and families through care transitions, psychosocial support, and advocacy in a fast-paced healthcare environment.

What You'll Do:
• Provide psychosocial assessments for residents and develop individualized care plans
• Support residents and families through adjustment to illness, long-term care placement, and end-of-life planning
• Coordinate discharge planning and safe transitions of care in collaboration with the interdisciplinary team
• Serve as a key advocate for resident rights, needs, and quality of life
• Provide counseling, emotional support, and resource navigation for residents and families
• Maintain accurate and timely documentation in compliance with regulatory standards
• Collaborate closely with nursing, therapy, and administration to ensure holistic resident care

What We're Looking For:
• Active Licensed Clinical Social Worker (LCSW) in Texas (or eligible for licensure)
• Experience in skilled nursing, long-term care, hospice, or healthcare setting preferred
• Strong communication, assessment, and crisis intervention skills
• Ability to work effectively in a multidisciplinary team environment
• Passion for advocacy, resident support, and improving quality of life
