Responsibilities:

• Develop and maintain relationships with local hospitals, healthcare providers, and other stakeholders to ensure effective communication and collaboration
• Coordinate patient admissions, discharges, and transfers to ensure a smooth transition between care settings
• Participate in discharge planning and care conferences to ensure that patients receive comprehensive care
• Collaborate with interdisciplinary teams to identify and address patient needs, improving overall care outcomes
• Provide education and support to patients, families, and caregivers on discharge plans and post-acute care options
• Maintain accurate records and reports, ensuring compliance with regulatory requirements and facility policies

Requirements:

• High school diploma or equivalent required; degree in a related field (e.g., healthcare administration, social work) preferred
• 2+ years of experience in a hospital or healthcare setting; experience as a liaison or care coordinator a plus
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and collaboratively as part of a team
• Strong organizational and time management skills, with attention to detail and follow-through
