Job Title: Certified Nursing Assistant (CNA) – Home Health

Job Summary

We are seeking a compassionate, reliable, and dedicated Certified Nursing Assistant (CNA) to join our private home health agency team. In this role, you will provide high-quality, one-on-one personal care and daily living assistance to clients in the comfort of their own homes. Your primary goal will be to promote our clients' independence, dignity, and well-being while ensuring a safe and clean environment.

Key Responsibilities

• Personal Care Assistance: Aid clients with daily personal hygiene, including bathing, dressing, grooming, toileting, and oral care.
• Mobility Support: Assist with transferring, ambulating, and positioning clients safely using proper body mechanics or assistive devices (e.g., gait belts, Hoyer lifts).
• Nutrition & Hydration: Prepare nutritious meals according to dietary restrictions, assist with feeding if necessary, and encourage adequate fluid intake.
• Health Monitoring: Measure and record vital signs (blood pressure, pulse, temperature, respiration) and monitor for any changes in the client’s physical or mental condition.
• Medication Reminders: Provide timely reminders for self-administered medications.
• Light Housekeeping: Maintain a clean and safe environment by performing light tasks such as changing bed linens, washing dishes, doing client laundry, and tidying up immediate living areas.
• Companionship & Emotional Support: Offer meaningful companionship, engage clients in cognitive activities, and provide empathetic emotional support.
• Documentation & Reporting: Maintain accurate, timely daily care notes and promptly report any significant changes in the client’s health or behavior to the supervising nurse or care coordinator.

Minimum Qualifications & Requirements

• Certification: Active and unencumbered Certified Nursing Assistant (CNA) license in the state of practice.
• Education: High school diploma, GED, or equivalent.
• Life Support: Current and valid CPR and First Aid certification.
• Transportation: Valid driver’s license, reliable personal vehicle, and active auto insurance (required for traveling between client homes).
• Background Clearance: Ability to pass a comprehensive criminal background check, fingerprinting, and a pre-employment drug screening.
• Health Screening: Up-to-date TB (Tuberculosis) clearance and any state-mandated immunizations.

Required Skills & Competencies

Technical & Professional Skills

• Basic Medical Knowledge: Proficiency in taking accurate vital signs and recognizing signs of medical distress or cognitive decline.
• Infection Control: Strict adherence to standard precautions, hand hygiene, and infection control protocols in a home setting.
• Patient Transfer Safety: Knowledge of safe lifting techniques and experience operating standard assistive mobility equipment.

Physical Requirements

• Ability to lift, push, pull, and carry up to 50 pounds .
• Frequent bending, kneeling, stooping, standing, and walking throughout the shift.
