Home Health RN Case Manager

Monday to Friday, 8:00 AM - 5:00 PM

Job Description

We are seeking an experienced Home Health RN Case Manager to provide care and manage cases in the Greensboro and High Point areas. The ideal candidate will have a strong background in case management and be comfortable traveling throughout the day to meet patients' needs.

Responsibilities

• Case management for a diverse patient load.
• Conduct assessments and develop care plans.
• Coordinate with interdisciplinary teams to provide comprehensive patient care.
• Document patient care activities using HomeCare HomeBase or similar systems.
• Travel to various locations in Greensboro and High Point to provide care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of case management experience required.
• Current RN license (temp or pending licenses accepted).
• Experience with HomeCare HomeBase preferred.
• 2 supervisory references required.
