POSITION SUMMARY

The Assistant Manager – Structured Day Program TBI/NHTD supports the daily operation and delivery of person-centered services to participants with traumatic brain injury and/or individuals receiving NHTD services. The position helps ensure a safe, supportive, and engaging program environment while coordinating activities and services designed to maintain and enhance participants’ functional abilities, independence, and community integration. The Assistant Manager supports treatment planning, interdisciplinary communication, participant documentation, attendance tracking, Medicaid billing documentation, and program activities in accordance with individual service needs, organizational requirements, and applicable regulatory standards.

DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS

• Supports the daily operation of the Structured Day Program and promotes a safe, comfortable, supportive, and engaging environment for all participants.
• Develops and maintains a monthly calendar of activities and events based on participants’ interests, needs, abilities, and individual goals, and posts the calendar at least five (5) days prior to the end of each month.
• Provides or coordinates assessment, training, supervision, and assistance related to self-care, medication management, task completion, communication, interpersonal skills, problem-solving, socialization, sensory-motor skills, mobility, and community transportation.
• Supports participants in developing and maintaining skills necessary for greater independence in the community, including money management, household management, community integration, volunteerism, and employment-related skills.
• Attends required treatment plan meetings and completes and submits all required documentation in accordance with program requirements.
• Maintains regular communication with participants’ treatment team members, including a minimum of quarterly contact, to support coordination and continuity of services.
• Monitors participant participation, progress, and changing needs and communicates relevant observations, concerns, and service updates to the appropriate supervisor and treatment team members.
• Completes accurate daily progress notes for all participants and ensures documentation is submitted within 72 hours of service provision.
• Maintains accurate daily attendance records for all participants in accordance with program requirements.
• Prepares required Medicaid billing documentation and submits all documentation to the supervisor no later than the seventh day of the following month.
• Implements individualized activities and services that support participants’ functional, behavioral, social, and community-living goals and provides opportunities to practice these skills in appropriate settings.
• Ensures participant records and program documentation are complete, accurate, timely, and maintained in accordance with organizational, Medicaid, and regulatory requirements.
• Performs other related duties as assigned to support the effective operation and quality of the Structured Day Program.
• Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.

• Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
• Meets all job requirements and stays in compliance.
• Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.

QUALIFICATIONS

• Education:

• Bachelor’s degree in human services, psychology, social work, or another related field required.

• Experience:
• • Minimum of two (2) years of experience developing and implementing day habilitation plans, providing vocational education services, providing residential habilitation service, and/or providing job coaching/supportive employment services required.
  • Specialized Training, Licenses, Certifications:
  • A valid NYS Personal Care Aide (PCA) certification in either Home Health Care Aide certification or Nurses Aide Certification is required. Must be maintained and remain in good standing.
  • Qualified candidates who do not currently hold a PCA certification may be considered if they sign an education agreement and obtain certification within four (4) months of hire.
  • Knowledge, Skills, Competencies, and Abilities:
  • Proficient writing skills with grasp of basic grammar and spelling
  • Computer skills with the ability to learn and use various applications
  • Must be efficient, well organized, and have excellent verbal and written communication and interpersonal skills
  • Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential
  • Knowledge of DOH and other relevant regulations
  • Other Requirements:
  • Valid Driver’s License and satisfy criteria to be an agency driver required

  WORK ENVIRONMENT

  This role is performed in-person at the program site with travel between FREE and community locations required. Transporting individuals in an agency vehicle and using wheelchair lifts is required.

  PHYSICAL REQUIREMENTS

  • Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).
  • Employee is required to effectively communicate both verbally and in writing.
  • Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets.
  • Employee is required to reach with hands and arms.
  • Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed.
  • Employee is required to be able to lift 50 pounds at a time.
