The LVN will primarily focus on monitoring patient symptoms through technology and telehealth visits. With excellent attention to detail, the LVN will monitor and document symptoms, health data, and current medical conditions. The successful LVN will be able to provide consistency of care with patients and work closely with the in-house clinicians to ensure positive outcomes. Additional responsibilities for the LVN will include:

• Collaborating with in-house clinical team on follow up plan with patients who are being monitored between visits using a wireless monitoring devices and mobile app.
• Documenting and reviewing patient charts with accurate diagnoses and current conditions and responses.
• Communicating regularly with patients on concerns, current conditions, and responses to treatments.
• Monitoring wireless device and app data for changes in responses and health outcomes. 
• Escalating critical issues to clinicians on patient health. 
• Documenting all interactions with patients in the company portal. 

Requirements:

• 2+ years of experience in primary care, family practice, geriatrics, and disease management.
• LICENSED LVN – license must be current and in good standing.
• Associates or Bachelor’s degree preferred. Coursework that is approved by the California Nursing Board.
• The ideal LVN will have good working knowledge of Medicare, chronic conditions management, remote patient monitoring, and quality measurements.
• Excellent listener and highly professional communication skills; able to discern concerns through regular communication
• Strong technology skills (MS Office, Google, and EMR systems)
• Highly organized, proactive, and able to juggle both administrative and clinical duties.
• A LVN that is able to think quickly, resolve problems professionally, and has a proactive and organized workstyle. 
• Familiar wit preventative physical examinations (consistent with HCC and Annual Wellness Visit criteria).
• Previous customer service or sales and marketing experience is highly desired!

Compensation:

• $24 - $27/hour
