Key Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to guide clinical interventions including manual techniques, therapeutic exercises, ambulation, modalities, assistive devices, and wound care
• Assess and adjust patient treatment plans based on progress, document all required information in the electronic documentation system, and accurately record treatment minutes per regulatory requirements
• Evaluate treatment outcomes for effectiveness and make referrals as appropriate; provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans; provide comprehensive discharge summaries per regulatory and clinical requirements
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students; participate in required meetings, trainings, in-services, and conferences

Required Qualifications

• Minimum of 1 year of physical therapy experience required; new graduates with completed Level II clinical experience (not solely Level I observation) may be considered
• Long-term care or rehabilitation setting experience required
• Active Massachusetts Physical Therapist state license required
• Adherence to APTA and state-required practice acts, code of conduct, and ethics standards

Preferred Experience / Skills

• Experience with electronic documentation systems in a clinical setting
• Familiarity with DME procurement and regulatory compliance in a long-term care environment
• Experience supervising physical therapist assistants or rehabilitation aides
