Responsibilities

• Conduct utilization management activities including chart review for medical necessity and appropriateness of services.
• Collaborate with an interdisciplinary team to coordinate care and ensure appropriate levels of care and timely discharge planning.
• Apply utilization review guidelines and payer policies to determine authorization requirements and coverage.
• Document decisions and clinical rationale clearly in patient records, ensuring accuracy and compliance.
• Provide on-call coverage as required and communicate care plans with physicians and families.

Requirements

• Active Registered Nurse (RN) license in good standing.
• Experience in utilization management, case management, or care coordination preferred.
• Knowledge of medical necessity criteria, payer guidelines, and authorization processes.
• Strong documentation and communication skills; ability to collaborate across disciplines.
• Ability to work effectively in a skilled nursing setting and manage on-call duties.
