b>Oncology Nurse - Transitions of Care An Oncology Nurse - Transitions of Care at Thyme Care will serve patients diagnosed with cancer by providing clinical assessments, monitoring health changes, coordinating care transitions, and educating members and caregivers over the phone. The role reports to the RN Care Team Lead, supports culturally competent care, gathers member and provider feedback for service improvement, and assists with administrative projects. The position may be remote or hybrid based in the Nashville office. First Three Months Complete training and become proficient with Thyme Care’s systems, tools, and partners. Build trusting relationships with members, using listening and empathy as core skills. Follow Care Team policies, procedures, escalation pathways, and documentation standards in all interactions. Identify and prioritize member needs to keep them safe in the community. Assist members with care coordination and management after admission. Coordinate discharge plans with hospital case managers and follow‑up care with providers. Monitor progress, provide updates, and create targeted support plans with the healthcare team in case conferences. Establish relationships with payers and providers to optimize care and prevent readmissions. Partner with non‑clinical team members to address social determinants of health such as food, transportation, and home support. Conduct telephonic assessments, including pain assessments and medication reconciliation. Ensure access to medications and appointments, providing referrals and support as needed. Perform virtual home safety evaluations and assess the need for durable medical equipment or supplies. Refer members to PT, OT, skilled nursing, palliative care, hospice care, etc., as appropriate. Be available for urgent clinical escalations and consult support. What Drives Success Member‑first mindset and personal motivation aligned with the company mission. Qualifications Active, unrestricted compact Registered Nurse (RN) license; willingness to obtain additional state licenses as required. ADN or BSN degree; BSN preferred. Minimum 5 years of nursing experience, including at least 3 years of oncology nursing or case management, and a Certified Case Manager (CCM) credential. Proficiency in clinical documentation and use of electronic health records. Spanish language fluency preferred (written and verbal). Benefits and Compensation Non‑exempt, full‑time position with an hourly rate of $45.67. Required OCN certification at hire or commitment to obtain within 2 years. Must be located within the lower 48 United States due to contractual PHI access limitations. Shift: 11:30 am–8 pm EST; up to 2 holidays per year, with 1x hourly rate and 2.5x holiday rate.
