Key Responsibilities

• Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete thorough medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities
• Delegate and direct discipline-specific therapy services for assigned patients
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific state practice acts and all applicable regulatory requirements, maintaining professional licensure in good standing

Required Qualifications

• Active Physical Therapist license in good standing in the state of Indiana
• Completion of all state-mandated continuing education requirements
• Ability to perform comprehensive patient evaluations and apply clinical reasoning to develop appropriate treatment plans and goals
• Commitment to patient and employee safety standards and compliance with organizational policies and procedures

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality initiatives
