Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and completion of student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements; maintain professional licensure in good standing
• Float to additional clinic locations as needed

Required Qualifications

• Active Physical Therapist (PT) license in good standing in the state of Indiana
• Minimum of 1 year of clinical experience required; new graduates will not be considered
• Ability and willingness to float to multiple clinic locations as needed
• Demonstrated knowledge of and adherence to discipline-specific professional code of conduct and state practice act requirements

Preferred Experience / Skills

• Experience in an outpatient therapy setting
• Familiarity with functional quality indicators, letters of medical necessity, and insurance authorization processes
• Active involvement in continuing education and professional development activities
