Case Associate, Foster Care and Adoption

New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan dedicated to serving children and families with medical complexities, disabilities, chronic illnesses and behavioral challenges. To assist us in achieving this mission, NAC is currently seeking a talented Case Associate to join the Specialized Foster Care Program. Title of Position: Care Associate, Foster Care Adoption Position Type: Full-time Pay Range: $43,000-$47,000; if bilingual $44,000-$48,000 Location: Services provided throughout NYC in the field/community Administrative Location: 825 7th Avenue, New York, NY, 10019 Department/Program: Foster Care Description of Responsibilities Supervise family visits in the office and in the community, acting as a visiting coach and documenting visits for electronic case records. Conduct foster home visits, visits to birth homes and other field visits, documenting visits for electronic case records. Assist social workers with the necessary phone contacts to arrange appointments and make referrals for concrete services. Attend Family Team Conferences, Family meetings, and any other scheduled multi-disciplinary case conferences, as necessary. Accompany and advocate for parents at appointments for public assistance, housing, and other necessary services. Visit schools to discuss educational progress of children with teachers and other school personnel. Assist with data collection for adoption home studies. Provide other assistance, as might reasonably be required, to support case management and permanency efforts for families with children in foster care.
