RN Trust Case Manager/LCP Contractor

Trust Case Manager – Contract Position (Remote) We are seeking a highly skilled RN Trust Case Manager to support beneficiaries of special needs trusts with compassionate, coordinated, and efficient case management services. This is a fully remote contract role with occasional travel, limited to no more than one onsite visit per year. The ideal candidate thrives in a dynamic environment where case volume and intensity fluctuate—high activity at case onset, lighter ongoing management, and intensive virtual nursing assessments required annually. Position Highlights Work Arrangement: Primarily remote (continental U.S.). Cases are managed virtually with annual follow-up assessments conducted via telephone or video platforms. Case Load Nature: Cases “wax and wane.” Expect intensive coordination at the start of new cases, periodic check-ins throughout the year, and comprehensive annual nursing assessments. Employment Type: Independent contractor (1099). No benefits provided. Compensation: Competitive pay aligned with case management rates in middle America. Bi-weekly payroll for all hours billed during the prior two-week period. Required Qualifications Excellent telephone and interpersonal skills with the ability to build trust and communicate effectively with beneficiaries, families, and providers. Excellent computer skills and proficiency with Microsoft Office is required. Strong organizational skills and the ability to manage multiple cases with varying levels of intensity. Knowledge of Medicaid (in general terms, with the understanding that rules are state‑specific). Experience and knowledge of Social Security Disability (helpful). Preferred Qualifications Current CCM (Certified Case Manager) certification strongly preferred. CLCP, CNLCP, or FIG-C (Certified Life Care Planner or related) certification highly desired. Actual experience in arranging/pricing home and vehicle modifications (assessment, coordination, and implementation). Pediatric case management experience, particularly with the following conditions: Cerebral palsy Hypoxic Ischemic Encephalopathy (HIE) ADHD Traumatic Brain Injury (TBI) Organ transplant Feeding disorders and other complex pediatric disabilities Location Preference Texas and New Jersey (or surrounding states) preferred. Coordinate care, services, equipment, and home/vehicle modifications. Serve as the primary point of contact for beneficiaries and families, providing guidance and advocacy. Maintain accurate case documentation and billing records. Work independently while collaborating with trust administrators, attorneys, and service providers.
