Golden Palms Rehabilitation and Retirement invites a full‑time Physical Therapist to join our in‑house therapy team. Responsibilities Effectively screen and evaluate patients with physical functioning disorders and/or wound care and develop an appropriate plan of care in accordance with all regulatory and clinical practice standards. Provide rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care in accordance with regulatory and clinical practice requirements. Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Provide consultation and counseling to patients, families, caregivers and other service providers related to physical disorders. Effectively communicate with supervisors and health team members regarding patient progress, barriers and treatment plans. Document all regulatory and clinically required information in the electronic documentation system and maintain patient records in accordance with company policy. Accurately record treatment minutes for services provided in compliance with regulatory requirements. Participate in required meetings such as patient‑care conferences, utilization review meetings and rehabilitation meetings. Participate in facility and company‑required trainings, in‑services and conferences. Assist in obtaining necessary DME or supplies as required. Supervise and support physical therapist assistants, rehabilitation aides and/or students. Qualifications Licensed Physical Therapist. The position is open to PTs at all experience levels. New graduates are welcome.
