FeldCare Connects seeks a self-motivated Occupational Therapist for home health in the San Jose area. The OT assesses ADLs, functional status, and home safety, then designs and implements a treatment plan to improve patient wellbeing. Responsibilities include leading patients through therapy, monitoring progress, and instructing families and care teams on therapy participation.
