HOURY SALARY: $26.00 - $27.50 Per Hour

FLSA STATUS: Non-Exempt, full-time

GENERAL SUMMARY:

The Health and Social Services Coordinator connects families with appropriate health, special needs, nutrition, social services, mental health, and parent involvement resources, supporting families in making informed decisions that improve their well-being. The position also assists with child recruitment by attending community events and conducting pre-screening and enrollment activities for migrant families.

 

COMPONENT MISSION

The general objectives of the Health Services component are to:

1. Provide comprehensive health services, including medical, dental, mental health, and nutrition services, to preschool children, including children with disabilities, to support their physical, emotional, cognitive, and social development.
2. Promote preventive health services and early intervention.
3. Provide families with information, resources, and support to help connect them with an ongoing healthcare system so children may continue receiving comprehensive health services after leaving the program.

 

The general objectives of the Social Services component are to:

1. Provide comprehensive social services, including referrals, outreach and recruitment, access to community services and resources, and connections to services that support children and families.
2. Promote positive child development and prevention services.

 

ESSENTIAL FUNCTIONS:

1. Provides MAOF families with emergency assistance and crisis intervention.
2. Refers children and families to appropriate health, mental health, nutrition, disability, court, and social services; assists families with scheduling appointments and completing necessary follow-up.
3. Conducts family assessments and develops family action plans to identify and address service needs.
4. Assists the Site Supervisor with planning and presenting parent trainings related to child abuse prevention, health, social services, and other relevant topics.
5. Assists with the distribution of health, nutrition, disability, and other program supplies.
6. Assists families with transitioning children into the public school system.
7. Conducts pre-screening and enrollment activities for migrant families, including health history interviews.
8. Participates in parent orientation at the beginning of the program year.
9. Assists with recruitment of children by attending community events and outreach activities.
10. Collects and reviews completed health history forms for children enrolled at assigned sites.
11. Educates parents regarding the importance of medical and dental screenings, examinations, follow-up care, and treatment.
12. Assists families with scheduling medical and dental appointments and follows up to ensure services are completed.
13. Maintains accurate medical and dental records for assigned children, including immunizations, referrals, and follow-up on identified health concerns.
14. Serves as a primary parent contact for medical and dental matters and documents all related communication and follow-up.
15. Meets regularly with site staff to coordinate health and social service activities with the education program.
16. Assists teaching staff with activities that help prepare children for medical, dental, clinic, or hospital visits.
17. Inspects first aid kits monthly and requests replacement supplies as needed.
18. Monitors and manages the food pantry at the Salinas Child Care Center.
19. Communicates effectively with parents and children in their preferred language and maintains confidential child and family records.
20. Participates in required in-service training and professional development activities.
21. Attends required meetings as designated by the Site Supervisor, Program Director, and Director of Operations.
22. Performs other duties as assigned.

 

MINIMUM REQUIREMENT:

• Bachelor’s degree from an accredited college or university in Child Development, Education, Social Services, or a related field.
• Minimum of two (2) years of experience working with low-income families and/or children, including case management experience.
• Bilingual in English and Spanish required.
• Ability to work a flexible schedule, including occasional evenings and weekends as needed.
• Must possess a valid California Driver’s License, reliable transportation, and current automobile insurance.
• Ability to understand and work with complex or conceptual information.
• Ability to work independently with minimal supervision.
• Strong written, verbal, organizational, and interpersonal communication skills.
• Proficiency with computers and standard office software.
• Ability to work effectively with ethnically, culturally, linguistically, and socioeconomically diverse populations.
• Ability to establish and maintain cooperative and professional working relationships with coworkers, supervisors, families, and the public.
• Ability to work collaboratively as part of a team and manage multiple priorities and changing assignments within established deadlines.

 

HEALTH REQUIREMENTS:

• Must meet applicable State health requirements, including completion of a pre-employment physical examination and Tuberculosis (TB) clearance at the time of employment and renewal as required.
• Must provide required immunization documentation for influenza, pertussis, and measles in accordance with applicable program and State requirements.

While performing the duties of this position, the employee is regularly required to use hands, communicate, sit, stand, walk, and reach with hands and arms. The position requires regular driving and travel to program sites, family homes, community locations, and other work-related locations. The employee may occasionally be required to lift and/or move up to twenty-five (25) pounds. Vision requirements include close vision and the ability to adjust focus.

 

WORK ENVIRONMENT

The work environment includes office, childcare center, community, and field settings. The employee may regularly interact with children, families, staff, community partners, healthcare providers, and members of the public and may be required to travel between assigned locations.
