Geriatric Social Worker Responsibilities & Duties

• Assess the psychosocial needs of elderly clients
• Develop and implement individualized care plans
• Advocate for the rights and needs of seniors
• Provide counseling and emotional support to clients and their families
• Coordinate services such as healthcare, housing, and legal assistance
• Facilitate support groups for seniors and their caregivers
• Collaborate with medical professionals and other service providers
• Educate clients and families on aging-related issues
• Assist with end-of-life planning and decision-making
• Maintain client records and documentation
• Evaluate the effectiveness of care plans and adjust as needed
• Navigate clients through Medicare, Medicaid, and other benefit programs
• Promote social and recreational activities for seniors

Requirements:

• Master's degree required.
• Valid license as an LCSW issued by the NYSED.
• Experience with assigned patient population and Utilization of PM Schedule and CWS.
• Experience treating concurring substance abuse and mental illness required.
• Membership in the NASW Academy is a plus.
• Prior experience in a hospital or community health agency is preferred.

• Additional language fluency and computer literacy preferred.

Salary: $75,000 - $85,000 (Commensurate with Experience)
