• Monday - Friday, no weekends, no holidays -- 40 hrs/week guaranteed
• 5 - 6 patients per day
• Home health setting -- adult caseload
• Sees patients across 4 - 8 different facilities
• 25 - 30 points per week
• Home health & OASIS experience required
• EMR: HCHB (preferred)
• Strong documentation & communication skills required
