Description

• Conduct evaluations for patients within both inpatient and outpatient environments and formulate tailored treatment plans.
• Deliver physical therapy interventions emphasizing mobility, strength, balance, and functional abilities.
• Engage and collaborate with nurses, physicians, case managers, and additional rehabilitation professionals to maximize patient outcomes.
• Educate patients and their families on therapeutic objectives, discharge processes, and home exercise routines.
• Document assessments, treatments, and progress in accordance with institutional and regulatory standards.
• Participate in patient care team rounds and interprofessional conferences to guarantee cohesive care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent from an accredited institution.

Certifications

• Required state licensure as a Physical Therapist
• CPR certification is preferred.
