Job Description: We are seeking dedicated and compassionate nurses to join our team in providing exceptional care to seniors across Colorado. As a Home Care Nurse, you will play a crucial role in delivering personalized care plans tailored to meet the unique needs of each client. Full & Part time positions available. Your responsibilities will include: Conducting comprehensive assessments of clients' health status and needs. Developing and implementing individualized care plans in collaboration with clients, their families, and other healthcare professionals. Administering medications, treatments, and therapies as prescribed. Monitoring clients' vital signs and health indicators. Perform nursing procedures such as oxygen support, ostomy and catheter care, medication management, and wound care. Providing assistance with activities of daily living, such as bathing, dressing, and grooming. Educating clients and their families on health management and disease prevention. Coordinate with physicians and other healthcare professionals to ensure optimal patient care. Maintain accurate and timely documentation of patient interactions and care provided. Supervises Certified Nurse Assistants (CNA); trains and delegates specific tasks per agency policy. Communicating effectively with clients, families, and interdisciplinary team members. Qualifications: Current RN license in the state of Colorado. Minimum of 3 years of nursing experience, preferably in home care or geriatrics. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Ability to work independently and collaboratively as part of a multidisciplinary team. Compassionate and patient-centered approach to care. Reliable transportation and willingness to travel to clients' homes. CPR certification (required). Experience with electronic health records (preferred).
