Community Living Manager

Provide overall management of the day-to-day operation of supported independent living residential services. Position requires on-call support via cell phone for staffing issues/conflicts related to scheduling and must have reliable transportation.

High School diploma or equivalent. Associates degree preferred.

Minimum 2 years' experience working with individuals with disabilities and/or related field; 1 year of supervisory experience minimum.

South Carolina driver's license and vehicle insurance; must have ability to utilize Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.). Bachelor's Degree preferred

Supervises direct support professionals for the assigned residence.

Ensures daily operation of residential sites is in compliance with the policies of UCP and DDSN and provide quality care, oversight and support to individuals served.

Creates weekly house schedule to ensure the daily activities and needs of individuals served are met and secures coverage for any shift during absences and vacations based on the rates of each home.

Ensure all home/agency operations systems are working properly, to include: van cell, cable, computer, fax, security system and phone.

Provide direct support to individuals living in residential and supported living.

Investigate and resolve any issues of misuse of agency equipment or theft of finances, to include credit cards, house petty cash and Cash on Hand.

Monitor and ensure compliance in all aspects of agency vehicle, to include: registration, insurance, repairs, regular maintenance, vehicle accidents and van log usage.

Report any emergency/crisis situation to supervisor and follow up by completing appropriate incident report as well as reports to SLED any violations of individual rights, abuse, neglect or exploitation within 24 hours.

Works cooperatively with all contract providers, vendors, and agency staff, meets routinely with Area Director.

Monitors and ensures all daily household cleaning chores are completed. Home is comfortable, safe and free from hazards.

Attends Quarterly Psychotropic Medication Review at least quarterly or as needed.

Collaborates with individuals served to create a weekly menu. Ensures that groceries and house supplies are purchased weekly in accordance with the menu and there is ample product/choice for individuals served.

Provides on-call support to staff regarding house scheduling/leave issues and safety or operational issues.

Screens and interviews staff in respect to the needs of the clients served. Assists HR with obtaining necessary information and ensuring completion of new-hire training for staff with AD or designee.

Collaborates with Community Mentor to ensure staff receive training to reach optimum potential in the field.

Ensures that all staff complete required training including: CPR, CPI, First Aid, and mediation administration.

Attends all in-services and trainings, as necessary.

Conducts CLC house meetings to ensure on-going skill development and compliance/understanding of all policy and procedures, at least monthly.

Provides direct supervision of all Community Living Coaches to include evaluations, coaching's and required annual trainings, with approval of AD or designee.

Provides quarterly wheelchair tie-down and van safety training to all staff.

Attends annual Residential Plan meeting and ensures staff follow all guidelines in the plan.

Inspects home at beginning of shift to ensure that home meets all safety requirements, checking for clutter and dangerous objects. Correct hazard/deficiencies or completes a Maintenance/Operation Work order or submits written report to supervisor outlining deficiencies by end of shift.

Uses appropriate body mechanics to lift, move and position individuals in residential and supported living. Also provides guidance for exercise and physical care for the individuals served.

Conducts evacuation drills, both fire (quarterly for CTH-II and annually for SLP/CTH-I) and severe weather (annually). Records results of each drill and corrects any findings.

Conducts monthly inspections of all safety equipment, to include: first aid kits, smoke detectors, emergency lights, emergency food supplies, flashlights, carbon monoxide detector and security system. Ensure emergency phone numbers are posted by any phone.

Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe weather and missing persons. Report any occurrence to Area Director.

Notifies Medical Service Coord/Area Director of all medical emergencies as outlined by policies and procedures.

Checks to ensure that all flammable materials or household supplies are clearly labeled, properly stored and safeguarded.

Checks to ensure that food is protected from contamination and spoilage. Checks freezer and refrigerator to ensure that food is properly dated, wrapped and/or covered. Corrects any problem and reports to AD for further corrective action.

Checks to ensure all appliances, exhaust hood utensils and cooking/work surfaces are in good working condition. Complete Work Order to correct any issue.

Ensures the appropriate disposal of garbage, daily sanitizing of trash containers and lids, hauling off of unnecessary items to prevent hazard within or around the home.

Checks plasticware, chinaware, glassware or other similar items for chips or cracks. Places in a box to be picked up and removed from inventory in accordance with policy/procedure of UCP Residential Services.

Monitors expenditures on a routine basis, dependent on the individuals' needs, to ensure funds are properly planned for, managed and accounted for on the Consumer Statement of Account, in accordance with licensing standards and UCP policy/procedure.

Ensures all client choices are provided and personal belongings are kept safe and available as well as logged completely on Personal Property Inventories as required by DDSN standards.

Reviews quarterly CFA statements with the individual and/or family/legal guardian, obtaining signatures from all necessary parties per DDSN standards.

Monitors and maintains all house petty cash, receipts and monthly reconciliations while staying within the approved annual budget.

Ensures menus are posted on a weekly basis, in accordance with client preferences and within guidelines of any dietary restrictions.

All Credit card purchases are logged, documented and monitored to ensure limits are within house budget. Distributions are turned in monthly and discrepancies are investigated and findings submitted to AD.

Maintains records in the home to meet local, state and federal guidelines.

Makes client related entries in daily logs that are legible, dated, authenticated by signature and positions, in ink and without use of symbols or abbreviations.

Records contacts with client's family or guardian.

Maintains appropriate documentation relating to health, safety and infection control.

Maintains records of van log data sheets and monitors to ensure they are completed correctly by all staff.

Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior.

Complies with policies/procedures to ensure that confidentiality requirements are upheld.

Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and families as defined by agency.

Demonstrates competence in handling medical emergencies in accordance with UCP policies and procedures.

Reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time.

Observes all other personnel policies and works closely with supervisor to ensure compliance.

Abides by code of ethics and serves as a good role model for clients.

Ensures services provided are in compliance with local, state/federal rules and regulations and UCP Residential Services in the area of supervision/accountability for each client served and documented.

Administers medications in accordance with all Medication Technician Certification.

Develop and maintain positive and productive relationship with individuals served, families, co-workers, and community members.

Maintain objectivity in position in order to set appropriate limits while working within the residential and supported living settings.

Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences.

Obtains annual Tb test and ensures results are signed by appropriate medical staff.

Other duties as assigned.
