Care Coordinator (53153) → Care Coordinator

Role Care Coordinator for a Birth to Three Program, supporting children ages 0-3 with developmental delays or disabilities and their families. Responsibilities Partner with families, service providers, and community resources to support child development. Conduct functional assessments and develop individualized, family-centered service plans. Coordinate and facilitate services aligned with care plans. Provide education, advocacy, and support to families. Maintain accurate documentation in compliance with regulations. Lead family-centered team meetings and collaborate with internal teams and community providers. Manage a flexible schedule based on client needs, including occasional evenings or weekends. Meet productivity and service delivery expectations. Qualifications & Skills Bachelor's degree in Social Work, Child Development, or related field. Experience working with families with special needs is preferred. Valid driver's license, reliable transportation, and ability to meet driving and insurance requirements. Strong organizational, communication, and documentation skills. Comfort working in home, office, and community settings. Ability to respond effectively in crisis situations.
