Physical Therapist Assistant - Santa Rosa, CA Location: SantaRosa, CA 95404 Setting: SkilledNursing Facility (SNF) Employment Type: Permanent Compensation: $48- $55 per hour About the Opportunity We are seeking a Physical Therapist Assistant to join asupportive skilled nursing facility (SNF) rehabilitation team in Santa Rosa,CA. Working under the supervision of a licensed Physical Therapist, you willcarry out individualized treatment plans and help patients progress towardmeaningful functional goals. Responsibilities Implement treatment plans established by thesupervising Physical Therapist. Provide therapeutic exercise, gait, balance,mobility, and functional training. Monitor patient response and communicateprogress to the Physical Therapist. Educate patients and caregivers within theestablished plan of care. Complete accurate and timely documentation. Collaborate with the rehabilitation andinterdisciplinary care team. Qualifications Graduate of an accredited Physical TherapistAssistant program. Active CA PTA license in good standing, oreligibility for licensures. Current BLS/CPR certification as required by theemployer. Strong communication, documentation, teamwork,and patient-care skills.
