- Care Manager - Intellectual Developmental DisabilityUSD 22.80 - 33.27 Hourly|Case Management in 175 College St, Battle Creek, MI 49037-3432, United StatesFull Time**Job Summary/Objective:**Care Managers are responsible for providing goal-oriented and individualized services and supports, maintaining record compliance, participating in competency improvement activities, receiving training, and performing other duties as required.**Essential Duties and Functions:*** Provides Case Management services, such as assessments, planning, linkage, advocacy, coordination, and monitoring in an effective/efficient manner.* Connecting customers with appropriate resources and services.* Visiting customers within their homes and/or community settings.* Documents customer progress using both electronic and/or hard copy record formats.* Coordinates and operates under the guidance of a master's level behavioral health clinician.* Actively supports culturally competent, recovery-base practices, person-centered planning as a shared decision-making process with the customer, who defines his/her life goals and is assisted in developing a unique path toward those goals.**Qualifications:*** Bachelor's degree in human services related field, required.* Licensure in the State of Michigan, preferred, LLBSW/LBSW.
