POSITION DESCRIPTION

The new physician will see patients in skilled nursing facilities and other long-term care settings. Providers work autonomously in the field and are remotely supported by a robust clinical and administrative team. A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support.

The duties of the role may include:

• provide high-quality complex geriatric care
• ensure engagement with facility leadership, other providers, patients, and families
• supervise nurse practitioners and/or physician assistants
• audit documentation for each supervised provider on a monthly basis
• collaborate with other healthcare providers as needed
• participate in the on-call schedule
• attend trainings and meetings as required

The new physician will also focus on quality outcomes including, but not limited to:

• care path compliance
• prevention of avoidable transfers
• decrease in polypharmacy
• costs of care

The practice uses an EHR on a mobile-based platform.

REQUIREMENTS

• Tennessee medical license
• DEA
• Board Certification, relevant field (Internal / Family Medicine, Geriatrics, etc.)
• experience supervising APPs
• 3+ years relevant clinical practice experience
