Description

• Assess patients in both inpatient and outpatient settings and craft personalized treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate with fellow healthcare professionals, including nurses, physicians, case managers, and rehabilitation specialists to achieve optimal patient outcomes
• Educate patients and their families about therapeutic goals, discharge procedures, and at-home exercises
• Document assessments, treatments, and patient progress within established facility and regulatory guidelines
• Engage in care team meetings and interdisciplinary conferences to ensure coherent and comprehensive care coordination

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are a bonus but not necessary
