Your responsibilities include gathering and documenting social history, assessing support systems, participating in interdisciplinary team meetings, coordinating admissions, transfers, transitions, and discharges, and providing individual and group training on rights and self‑advocacy. The role operates under moderate supervision with limited latitude for independent judgment. Essential Job Functions Attends work regularly and may be required to work specific or rotating schedules, overtime and on‑call duties. Interviews patients, representatives, and agencies to assess psychosocial, economic, and physical needs. Oversees communication between patients, representatives, agencies and treatment providers, ensuring liaison functions are performed. Provides guidance on guardianship, living options, end‑of‑life planning, and rights in compliance with state and federal regulations. Coordinates the rights and capacity assessment processes, reviews documentation, and assigns staff for admissions and other transitions. Designs and facilitates individual and group trainings on rights, voting, citizenship, self‑advocacy, and decision‑making. Participates in patient and family meetings, presenting facility processes. Supports disaster response, recovery, or COOP activation as needed. Knowledge, Skills & Abilities Knowledge of intellectual and developmental disabilities, mental health, social work standards and case management principles. Skill in accountability, limited financial responsibilities, and property management. Proficient interviewing, assessment, planning, intervention, evaluation, case management, mediation, counseling, information and referral, problem solving, supervision, consultation and advocacy. Ability to practice in accordance with Texas State Board of Social Work Examiners Code of Conduct and National Association of Social Workers Code of Ethics. Effective written and verbal communication in individual and group settings. Registration, Licensure & Certification Requires maintenance of licensure with the Texas Board of Social Work Examiners. Initial Screening Criteria Baccalaureate degree in Social Work, Master’s in Social Work, or Clinical Social Work. One year of direct experience with persons with mental health issues, OR completion of State Hospital internship program. Additional Information Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks. Males 18‑25 must register with Selective Service. Minimum age is 18; youth center applicants must be 21. Flexibility for overtime and extended hours may be required.
