Acute Care Clinical Pharmacist Specialist Hematology-Oncology

Acute Care Clinical Pharmacy Specialist Primary City/State: Tucson, Arizona. Department: Pharmacy–Hospital. Shift: Day (Monday–Friday, rotating weekends and holidays). Category: Pharmacy. In this role, you will provide pediatric oncology unit‑based decentralized clinical services such as order verification, medication review, pharmacokinetic monitoring, and renal dosing. You may also cover pediatric satellite/centralized staffing as needed. Responsibilities Critically evaluate disease‑state management and adjust therapy to improve patient medication regimens and ensure cost‑effective practice. Serve as the drug information expert for the specialty; provide detailed drug information and education to healthcare team, patients, and community. Review and evaluate medication orders for appropriateness and completeness based on patient data. Apply continuous quality improvement principles, including data collection, documentation, and error trending. Participate in local, system‑based committees and communicate outcomes to stakeholders. Serve as a primary preceptor to residents and students; meet ASHP preceptor qualifications and, if required, act as Program Director for the pharmacy residency program. Qualifications Doctorate in Pharmacy and completion of a PGY1 residency program (or equivalent 3 years of progressive health‑system clinical experience). Board certification through the Board of Pharmaceutical Specialties (active and maintained) is required. Licensed pharmacist in the state of practice; must obtain licensure within 90 days of hire. Strong organizational, problem‑solving, and communication skills. Experience with medication management software, pharmacy system software, and automated dispensing systems. Preferred: PGY2 residency or PGY1 plus >3 years health‑system experience; Board certification in a relevant specialty; expert‑level knowledge of assigned area; experience precepting residents.
