Pride Health is hiring an RN - Home Health/Hospice to support our client's healthcare facility in Tucson, AZ. Schedule: Monday-Friday | 8:00 AM-5:00 PM | 40 Guaranteed Hours/Week | On-Call/Weekend: No Location: Tucson, AZ; Coverage areas include Tucson, Oracle, San Manuel, Mammoth, Oro Valley, Saddlebrooke, Marana, Picture Rocks, and Casas Adobes; Mileage reimbursed between visits from first patient to last patient per contract company. Job Type: Contract Contract Length: 8 Week/s Pay Range: $52-$54 *Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Manage and coordinate patient care as an RN Case Manager with a caseload of 18-22 patients. • Complete routine, PRN, and end-of-life visits, as well as patient admissions. • Assess patient needs and develop individualized plans of care. • Complete accurate and timely patient documentation and follow-up.

Requirements:

• License(s): Active Nursing License • Certification(s): CPR • Years of experience: 1+ • Skills/Specialties/Technology: Hospice experience preferred
