Responsibilities
- Coordinate and manage comprehensive care plans for hospice patients, ensuring their needs are met with compassion and precision.
- Conduct regular assessments of patients' conditions, adjusting care plans as necessary to provide optimal support.
- Collaborate with an interdisciplinary team to deliver holistic care, fostering a supportive environment for patients and their families.
- Provide education and guidance to patients and families, empowering them with knowledge and confidence in managing care at home.
- Administer medications and treatments as prescribed, prioritizing patient comfort and symptom management.
- Document patient progress and care activities accurately and in a timely manner, maintaining clear communication with the care team.
- Advocate for patients' needs and preferences, ensuring their voices are heard and respected in all care decisions.
Qualifications
- Current Registered Nurse (RN) license in the state of New Jersey.
- Experience in hospice or palliative care, with a focus on patient-centered approaches.
- Ability to develop and manage individualized care plans that prioritize patient comfort and quality of life.
- Proven track record of collaborating effectively within an interdisciplinary team.
- Strong communication skills to educate patients and families on care options and symptom management.
- Ability to conduct thorough patient assessments and adjust care plans as needed.
- Proficiency in maintaining accurate and timely documentation in compliance with healthcare standards and regulations.
