Assess patient groups and monitor progress, adjusting therapy plans to meet individualized goals. Co‑lead interdisciplinary care activities, collaborating with other professionals to coordinate patient care. Educate patients and caregivers on treatment strategies, home exercise programs, and safety planning. Document patient status, goals, and outcomes in accordance with department policies and regulatory requirements. Ensure adherence to approved treatment protocols and maintain patient-centered, evidence‑based care.
