As a Care Coordinator, you’ll serve as the connection between our clients, families, caregivers, and nurses, helping ensure the right care is delivered at the right time. What You’ll Do Coordinate daily schedules for caregivers and nursing staff Build and maintain strong relationships with clients and families Communicate schedule changes and care updates promptly Support and assist field staff with day-to-day needs Investigate and resolve client and employee concerns Maintain accurate documentation and reporting Partner with clinical and operational teams to ensure quality care Assist with ongoing quality improvement initiatives Why Phoenix?
