Job Description
Title: LPN - POST-ACUTE SKILLED NURSING REHAB

Schedule: 12-Hour Night Shift

Assignment Details:
- Contract Length: 4 weeks
- Guaranteed Hours: Not specified

Requirements:
- Licensure: LPN
- Certifications: IV Certification preferred; BLS
- Years of Experience: Minimum 1 year required, with a preference for experience in skilled nursing rehab
- EMR: EPIC experience from within the past 2 years is required

Roles & Responsibilities:
- Provide direct patient care in a post-acute skilled nursing rehab setting
- Administer medications and treatments as prescribed
- Perform assessments and document patient progress
- Collaborate with the healthcare team to optimize patient outcomes
- Assist with procedures such as phlebotomy and EKGs as needed
- Support patients in activities of daily living and promote their independence

Compliance / Notes:
- Must be able to work all holidays
- Reliable transportation is strongly recommended
- Dementia capable care certification preferred
- Orientation is bi-weekly on Mondays
- Background check including nationwide and county court searches, FACIS3, and sex offender check within 18 months of start date is required
