Compassionate Hospice RN Case Manager

UnitedHealth Group is seeking a Hospice Registered Nurse Case Manager to deliver the Plan of Care for hospice patients in home settings. You will evaluate, coordinate, and implement treatment plans with the care team to promote physical, psychosocial, and spiritual well-being. Requirements include an active RN license in the practice state, a valid driver's license and reliable vehicle (or access to transportation), and CPR certification or ability to obtain within 90 days. #J-18808-Ljbffr UnitedHealth Group
