Job Title Contingent Clinical Care LVN/LPN PT - MC Location Chicago, IL Job Description Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources. Conduct medication reviews to identify potential side effects and drug interactions. Accurately document patient medical history using electronic health records. Use excellent communication skills to interact with patients and healthcare professionals. Critical Information Work schedules will be created in 6-week blocks following the initial 3 weeks of training/orientation. A typical work week will be two or three weekdays and every other weekend. This is an in-person, patient-facing role. Education / Licenses Full, active and unrestricted license as a Licensed Practical or Vocational Nurse. Previous experience in Primary Care, Urgent Care, or Emergency Room settings. Proficiency with electronic medical record systems; Epic experience preferred.
