Registered Nurse - Case Management - Hospital

Job Description

Job Summary:
Seeking an experienced Registered Nurse for hospital case management. This role involves coordinating patient care and ensuring compliance with clinical and regulatory standards to optimize patient outcomes.

Responsibilities:

• Assess patients' needs and develop individualized care plans.
• Collaborate with multidisciplinary teams to coordinate discharge planning and care transitions.
• Review and authorize DME requests and implement Plan of Care.
• Ensure compliance with CMS regulations and hospital policies.
• Maintain accurate documentation in the EPIC electronic health record system.
• Pass a mandatory EKG exam and complete required modules during orientation.

Qualifications:

• Hold an active Registered Nurse license.
• Possess at least 2 years of hospital experience, including ICU, MS, PICU, SDU, PCU, IMC, or Obs specialties.
• Have recent acute hospital experience and previous charge nurse responsibilities.
• Be BLS certified and experienced with EPIC EMR.
• Demonstrate knowledge of regulatory requirements and discharge planning processes.
• Be flexible with scheduling, available for 5x8 day shifts, Monday through Friday.
