Overview
Physical Therapist needed for a travel assignment in a skilled nursing facility located in Albuquerque, New Mexico. Start date: 10/05/2026. Assignment duration: 14 weeks. Weekly pay: $2,260 to $2,357 per week. Guaranteed hours: 35 per week. Shift: 5x8 days, 08:00–16:30.

Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for residents in a skilled nursing facility setting.
- Implement evidence-based therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
- Monitor progress, document goals and outcomes, and adjust the plan of care as needed in coordination with the interdisciplinary team.
- Educate residents and families on exercises, safety, fall prevention, and home exercise programs; promote adherence and understanding of goals.
- Collaborate with nursing and therapy colleagues to support discharge planning, transfers, and goal attainment; ensure safety and quality of care.
- Maintain accurate and timely documentation in compliance with regulatory and facility requirements.

Requirements
- Active physical therapist license in the United States.
- Ability to work effectively in a skilled nursing facility environment with a geriatric population.
- Strong assessment, problem-solving, and documentation skills.
- Excellent communication and teamwork abilities; comfortable coordinating with an interdisciplinary team.
- Prior experience in a geriatric or SNF setting is preferred but not required.
