Manager Clinical Pharmacy-Kidney Transplant

Job Description In this role, as Clinical Pharmacy Manager, you will work closely with the Lead Clinical Pharmacy Manager, other Clinical Pharmacy Managers, and Operational Managers, to implement quality patient care services. You will provide collaborative pharmaceutical care to patients while working in conjunction with physicians, nurses, and other health care professionals. The Clinical Pharmacy Manager provides direct patient care and conducts teaching and research activities to promote optimal, cost‑effective patient care. Responsibilities Maintain a liaison relationship with the kidney transplant services through participation in patient care rounds/clinic and involvement in joint projects. Identify problems/issues in specific patients and promote rational drug use on a case‑by‑case basis using evidence‑based therapies (includes therapy selection and monitoring and assessment of outcomes of therapy). Provide drug information to providers covering the service and patients covered by the Kidney Transplant Service. Report adverse drug reactions for patients on service. Participate in multidisciplinary committees. Provide patient education and transitions of care. Develop, implement, manage and report results pertinent to continuous quality improvement and expense reduction programs related to the patients serviced. Conduct medication use evaluations. Develop treatment protocols/guidelines and clinical pathways. Implement other products/services that fulfill these goals. Required Criteria Completion of PGY2 Residency in Solid Organ Transplant. Completion of a Bachelor of Science degree in Pharmacy or PharmD. New York State Pharmacy License or eligibility to obtain license within 3 months.
