Humana/iCare is seeking an RN Care Manager to join the Family Care Partnership (FCP) program in Milwaukee and surrounding counties. You will perform home visits, complete nursing assessments, and ensure members access to appropriate services. The role requires Wisconsin RN licensure and a minimum of two years in health care, with preferred BSN and prior home health or case management experience.
