Collaborative Care Model: Join a tight-knit, established team of four daytime physicians and two daytime APPs, offering a supportive environment for professional growth. Lead Patient-Centered Care: Diagnose and treat acute and chronic conditions, ensuring every individual receives a clear path toward recovery and long-term wellness. Manage the Clinical Journey: Drive the full continuum of carefrom initial assessment and diagnostic interpretation to preventative screeningsto identify health risks early. Create Collaborative Treatment Plans: Coordinate seamlessly with physician partners and a robust network of specialists, ensuring patients never navigate their health journey alone. What minimum qualifications you'll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Education: Master's degree of Nursing required.
