In this role, you will be a vital anchor for our healthcare team, assessing and monitoring all aspects of our individuals' medical and psychiatric care. What You'll Do (Responsibilities) Collaborative Care: Partner with community healthcare providers, nutrition services, and behavior specialists to deliver high-quality, holistic care. Care Coordination: Oversee and coordinate all healthcare appointments, ensuring thorough and timely follow-ups. Team Education: Train, empower, and delegate medication administration and nursing tasks to the Direct Support Team in accordance with state licensing and Mosaic policies. Medical Record Management: Maintain, review, and audit medical records to monitor health concerns and document the delegation of nursing tasks. Schedule & On-Call Structure Primary Hours: Generally day hours. On-Call Rotation: Every 3rd week (primarily phone-based for emergencies; in-person response is rare). On-Call Support: You are never on an island—you will be backed by a strong support network, including a team of supervisors, QIDPs (Qs), and an RN Supervisor. Who Will Thrive in This Role? Requirements Bachelors of Science in Nursing, or the degree required based on State and agency definitions. Minimum of two years of general nursing experience required. Work requires frequent physical activity including extended periods of standing, walking, and bending with occasional periods of sitting, kneeling, climbing, stooping, crouching, squatting and balancing. Work also requires constant reaching between knee and shoulder level as well as frequent reaching below knee level and overhead. Work requires occasional independent lifting up to 25 pounds, frequent push/pull up to 40 pounds of force and the ability to safely transfer 50 pounds. #J-18808-Ljbffr
