About the Role:

We are seeking an experienced LPN License Practical Nurse to join our team of dedicated healthcare professionals. As an LPN License Practical Nurse at Cliveden, you will play a vital role in providing short-term post-acute rehabilitation and long-term skilled nursing care to our patients. Responsibilities:

• Provide direct patient care to assigned residents, including medication administration, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Monitor and report changes in patient condition to the nurse manager or Charge Nurse

• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

• Maintain accurate and thorough records of patient care and treatment

Requirements:

• LPN license (Practical Nurse) in the State of Pennsylvania
