Patient Navigator – Stony Brook CPMP Ambulatory Location: Lake Grove & Commack, NY Schedule: Full Time Days/Hours: Monday – Friday, 7:30 AM – 8:00 PM; Saturday – Sunday, 8:00 AM – 4:00 PM; hours vary Pay Range: $19.78 – $24.72 (dependent on experience and education). Compensation discussions occur during hiring. Summary The Patient Navigator will assist the departments with coordination of the daily operations of the front information desk at the Commack or Lake Grove location, perform various special project duties to ensure efficient workflow and patient care, and assist with patient transportation. Occasional travel between Commack and Lake Grove is required. Job Duties & Essential Functions Greets all scheduled patients upon arrival. Assesses telephone calls, determines urgency, and refers urgent queries to physician or nurse immediately. Uses Staxi chairs to assist patients from vehicle to appointment and back. Tracks all new patient inquiries including demographics, appointment date, and provider. Follows up with newly scheduled patients to assess experience. Performs receptionist duties: answers, screens, routes calls; takes appropriate messages and ensures delivery. Facilitates patient flow by notifying providers of arrivals, delays, or backlogs and coordinating with staff. Uses Electronic Medical Record (EMR) and other systems for appointment navigation. Tracks visitors in the e-log for contact‑tracing as required. Maintains clean and organized reception area. Provides additional coverage for evening hours and weekends, as needed. Coordinates with building security and management to ensure patient safety, offers wheelchairs, and ensures drop‑off area is clear and accessible. Ensures all ADA equipment is functional and hand‑sanitizing station is working and stocked. Follows HIPAA guidelines at all times. Multitasks effectively and professionally. Retrieves and ensures delivery of all incoming mail and packages to each specialty practice. Performs other duties as assigned by management. Required Education & Qualifications High School diploma or GED. Strong organizational and communication skills (verbal and written). Excellent typing skills and friendly telephone etiquette. Preferred Qualifications Graduated from an accredited Medical Office Administration program. At least 6 months experience as a receptionist. At least 1 year of clerical or customer‑service experience. Experience using an EMR and a practice‑management system. Bilingual in Spanish. Physical Demands The position requires regular communication with patients, staff, and medical providers, constant movement within the office to assist patients and staff, and other physical demands as outlined by management.
