Wellness Case Manager & Intake Coordinator
Endeavors Inc.
Endeavors seeks a Case Manager/Intake Coordinator to assess eligibility and enroll CBP employees and eligible family members into the CBP Workforce Wellness Program. This role will serve as a key contact, conduct comprehensive eligibility screenings, and ensure timely access to program resources. You will deliver comprehensive case management in office or community-based settings, coordinating care with clinicians and community partners while maintaining detailed case records and ensuring #J-18808-Ljbffr Endeavors Inc.
