Case Planner

JCCA is currently seeking dedicated Case Planning staff to join our team in supporting the Foster Home Services Medically Fragile program. As part of this role, you will collaborate closely with our Sociotherapist team to ensure the safety and well-being of every child, with a primary emphasis on comprehensive case planning. In this program, children in foster care with significant medical needs are placed in homes where caregivers receive specialized training to administer life-sustaining medications, treatments, and equipment, accompany them to various appointments, and provide support with daily living activities.

Essential Generic Job Functions

• Complete FASP, Initial Review Summary, Kinship Home Study/Summary
• Attend family conferences (72-hour, 30-day, Trial and Final Discharge, SPR)
• Coordinate family and/or sibling visits as clinically indicated.
• Develop visiting plan with family and provide feedback, assess and teach parenting skills
• Assess service needs and assist family in creation of a service plan.
• Review SPR planning goals monthly.
• Complete case contact portion of TPR summary.
• Complete statistical change forms to document case changes.
• Complete 853C, goal changes and plan amendments.
• Complete adoption summaries.
• Perform ongoing clinical assessment of child
• Prepare individual IL monthly contact and documentation
• Crisis intervention as needed
• Conduct home visits to parents
• Conduct home visits to foster parents. Adhere to contact mandates with the child, foster parent and parent.
• Maintain progress notes and complete monthly contact grids.
• Complete incident reports.
• Review step charts and behavior plans with Sociotherapist
• Prepare court reports (writing and presenting them).
• Participate in foster parent support group
• Design and implement behavior plans in consultation with Sociotherapist
• Maintain case record
• Follow-up with community resources (as necessary)
• Group Independent Living Activities may be required
• Group parent activities may be required

Qualifications

• Bachelor's Degree in Social Work, Human Services or related field is required. Master's degree preferred.
• Experience serving youth and families is required; internship experience is acceptable
• Should demonstrate the ability to focus treatment and planning with children and families towards permanency.
• Must demonstrate the ability to collaborate with client and teams that are culturally and professionally diverse.
• Excellent writing and follow-up skills are required.
• Bilingual English/Spanish is highly preferred

Hourly Rate: $27.47 - $29.71
