Responsabilidades Esperanza Health Centers está contratando una enfermera registrada (RN) para apoyar y coordinar las necesidades de atención clínica de los participantes de PACE a través de una evaluación de enfermería integral, monitoreo, coordinación de cuidados y comunicación de preocupaciones clínicas. Director del sitio PACE. Recopilar datos de los participantes y completar los informes, evaluaciones y documentación requeridos de acuerdo con los requisitos regulatorios y organizacionales. Participar en actividades de mejora de calidad, aseguramiento de calidad y cumplimiento, incluyendo auditorías de registros médicos e iniciativas de acción correctiva. Mantener la preparación para encuestas y auditorías federales, estatales, de CMS y organizacionales. Coordinar la atención y facilitar la comunicación entre participantes, cuidadores, hospitales, instalaciones de cuidado a largo plazo, proveedores contratados y socios comunitarios para garantizar la continuidad de la atención. #J-18808-Ljbffr Esperanza Health Centers
