About Saint Mary's Behavioral Health Center Saint Mary's Behavioral Health Center (SMBHC), LLC is an evidence-based, client-centered treatment program providing compassionate, individualized care for adolescents with behavioral health needs. Our programs include Psychiatric Residential Treatment, Partial Hospitalization (PHP), and Intensive Outpatient Services, creating a continuum of care designed to support each client's unique treatment journey. We are seeking a compassionate and experienced Medical/Family Nurse Practitioner (FNP/NP) to provide consultative medical services within our adolescent Psychiatric Residential Treatment Facility (PRTF). This role is ideal for a provider who enjoys collaborative care, complex clinical assessment, and supporting adolescents with co-occurring medical and behavioral health needs.
Position Summary The Part-Time Medical/Family Nurse Practitioner provides medical evaluation, treatment recommendations, and ongoing medical oversight for adolescent residents receiving psychiatric residential treatment. The Nurse Practitioner will evaluate residents with chronic medical conditions, newly identified medical concerns, medication-related medical issues, and acute non-emergent health concerns referred by psychiatric providers, the Medical Director, or nursing staff. This position functions as a consultative medical provider and does not serve as the primary care provider for all residents. Routine admission physicals, annual wellness visits, and primary care services are completed externally unless specifically assigned. The NP collaborates with the interdisciplinary treatment team to ensure residents' medical needs are appropriately assessed, managed, monitored, and integrated into their overall treatment plan. The Nurse Practitioner practices within the scope of Georgia law, facility policies, Georgia DBHDD regulations, CMS Conditions of Participation, and applicable accreditation standards.

Essential Responsibilities
Medical Consultation & Evaluation
- Perform focused medical evaluations upon referral from the Medical Director, psychiatric providers, or nursing staff.
- Evaluate residents experiencing changes in medical status requiring additional assessment.
- Assess newly identified medical concerns discovered during psychiatric treatment.
- Evaluate and manage chronic medical conditions requiring monitoring while residents are in treatment.
- Conduct focused physical examinations as clinically appropriate.
- Determine when referral to a primary care provider, specialist, urgent care, or emergency department is necessary.
- Provide recommendations to support medical stability and continued participation in residential treatment.
- Provide evaluation and treatment recommendations for non-psychiatric medical conditions within the NP scope of practice, including but not limited to:
- Asthma
- Diabetes
- Hypertension
- Gastrointestinal disorders
- Allergic conditions
- Dermatologic conditions
- Minor infections
- Headaches
- Menstrual concerns
- Minor musculoskeletal injuries
- Other chronic or newly diagnosed medical conditions
- Develop, implement, and monitor Health Monitoring Plans (HMPs) as clinically indicated.
- Monitor residents' ongoing medical needs, symptoms, treatment response, and changes in health status.
- Review medical histories, nursing documentation, and relevant clinical information to support informed medical decision-making.
- Order laboratory studies and diagnostic testing as clinically appropriate.
- Review and interpret laboratory results.
- Monitor abnormal lab findings and provide follow-up recommendations.
- Coordinate repeat testing, referrals, and additional medical evaluation when needed.
- Communicate clinically significant findings with the treatment team.
- Prescribe and manage medications for non-psychiatric medical conditions within the NP scope of practice.
- Monitor medication effectiveness, safety, and potential adverse effects.
- Collaborate with psychiatric providers regarding potential medical complications or side effects associated with psychotropic medications.

- Assist psychiatric and nursing staff in determining medical appropriateness for continued residential treatment.
- Recommend transfer to a higher level of medical care when clinically indicated.
- Participate in medical record reviews as requested.
- Assist with regulatory compliance related to medical services.

- Support accreditation and licensing readiness activities.
- Master's degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program.
- Current Georgia Registered Nurse (RN) license.
- Current Georgia Advanced Practice Registered Nurse (APRN) license.
- National certification as:
- Family Nurse Practitioner (FNP-C/FNP-BC), or
- Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
- Current CPR/BLS certification.
- Current DEA registration (if prescribing controlled substances).
- Two or more years of Nurse Practitioner experience preferred.
- Experience in pediatric or adolescent medicine.
- Experience in behavioral health, psychiatric, or residential treatment settings.
- Experience caring for patients with co-occurring medical and behavioral health conditions.
- Knowledge of medical considerations related to psychiatric medications.
- Experience with EMR documentation and clinical compliance standards.
- Strong clinical assessment and diagnostic skills.
- Knowledge of adolescent medicine and common pediatric medical conditions.
- Ability to recognize medical conditions requiring escalation of care.
- Strong understanding of medical complications associated with behavioral health treatment.
- Excellent communication and interdisciplinary collaboration skills.
- Ability to work independently while exercising sound clinical judgment.
- Proficiency with electronic medical record systems.
- Ability to perform focused physical examinations.
- Ability to respond to medical consultations onsite.
- Ability to walk throughout a residential treatment facility.
- Ability to lift up to 25 pounds as needed.
- Part-time position within an adolescent Psychiatric Residential Treatment Facility.
- Flexible scheduling based on facility needs and resident referrals.
- Work environment includes adolescents with behavioral health disorders and co-occurring medical conditions.
- Occasional exposure to communicable diseases and behavioral health emergencies.
- Provide timely and appropriate medical evaluations for residents with identified medical concerns.
- Deliver evidence-based medical care within the scope of practice.
- Maintain accurate, compliant, and timely documentation.
- Collaborate effectively with psychiatric providers and interdisciplinary team members.
