Job Description Registered Nurse (RN -CN 1) Medical Step Down Unit, ONA Medical Step-Down Unit (MSD)- 12-bed unit Population Served: Medical Step-Down Unit (MSD) provides an intermediate level of care between the Intensive Care Unit (ICU) and the Medical-Surgical Unit. Most commonly seen diagnoses include but are not limited to our Rapid Response patients that have deteriorated on the medical surgical floors, Renal failure, advanced respiratory illness, GIB, alcohol withdrawal, pulmonary hypertension, sepsis, medication overdoses, end-of-life care, and mental status changes of unknown origin. Bedside procedures- bronchs, desensitization, high-risk medication initiation, and titration. Medical Step Down nurses provide care as follows but are not limited to: critical care assessments, patient education, cardiac monitoring, hemodynamic monitoring, medication gtts, and trach to vent patients. This unit requires nurses to be oriented in the MICU, ACLS certified, and attend a 15-week intensive course for critical care nursing. There is a collaboration of interdisciplinary teams which is essential to the care provided to these patient populations. The goal of the unit is to provide a critical care environment to stabilize sick patients with chronic and complex needs and promote wellness. MSD patients have transferred out of MSD to LTAC (Long term acute care) environment or to the med/surg floor. The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social, and nursing sciences to assess, plan, implement, and evaluate patient care. The care is culturally based and age-specific. Prepares, implements evaluates, revises, and coordinates interdisciplinary plan of care for each assigned patient in a collaborative effort with the other healthcare team members and patient/family within the time frame indicated by the unit's standards of practice. Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patient's care, and demonstrates the ability to establish priorities of care based on the patient's assessment and needs. Gives clear instructions to patients/family regarding treatment, and involves family/guardian in the assessment, initial treatment, and continuing care of the patient. Identifies and responds appropriately to different patient needs resulting from behavioral health problems, unique psychological needs, or those associated with religious/cultural norms. Demonstrates critical thinking, accountability, flexibility, and a cooperative attitude in meeting the needs of the unit. The nursing process is accurately and concisely documented, including evaluation of treatment and in accordance with hospital policy and procedure. Acts as a resource person and mentor for new personnel, floater, and/or supplemental staff working on the unit.
