SNF PT Opportunity – Albuquerque, NM

Ardor Health Solutions is seeking a Physical Therapist for a Skilled Nursing Facility travel assignment in Albuquerque, New Mexico .

This is a strong opportunity for a PT who enjoys working with adult and geriatric patients in a rehabilitation environment. You'll help patients regain strength, balance, mobility, and independence following hospitalization, surgery, illness, or injury.

Position Highlights

• Gait, balance, strengthening, and mobility-focused treatment
• Collaborative interdisciplinary rehab team

Qualifications Required

• Graduate of an accredited Physical Therapy program
• Current New Mexico PT license or ability to obtain prior to start
• Current BLS certification
• Strong evaluation and treatment skills
• Comfortable working with adult and geriatric patients
• Strong documentation and communication skills
• SNF or geriatric rehabilitation experience preferred

What You'll Do

• Complete PT evaluations and reassessments
• Develop individualized plans of care
• Provide gait and balance training
• Address strength, endurance, transfers, mobility, and functional limitations
• Provide therapeutic exercise and functional training
• Help patients improve safety and independence
• Provide fall-prevention education
• Educate patients and caregivers
• Collaborate with PTA, OT, COTA, SLP, nursing, and other members of the care team
• Assist with discharge planning and recommendations
• Complete timely clinical documentation
