Schedule:

Coverage Type: Call only
Schedule Type: Call only
Call Type: Beeper
Dates of Need: Varies, 1:3 call rotation
Shift Times: Thursday at 7AM MST - Monday at 7AM MST
Lunch: Not applicable
Provider Minimum Shifts Required: 1:3 call rotation every month ongoing
Guaranteed Hours per Day: N/A (call)
Hours Worked for OT to apply: N/A (call)
Rates to be Filled Out on Bid: 24 Hour Beeper Call: Orientation, 24 Hour Call, Call-Back

Credentialing Requirements:

Current CV (must be uploaded to bid)
License: Active or IMLC
DEA Required: Yes
Boards: BC only
Skills/Certs: N/A
Other: N/A

Additional Details:
Provider will use our secure paging service Thursday at 7AM MST - Monday at 7AM MST while in a 1:3 rotation
